Required Seasonal Forms
All skiers, past and present, should be aware of our registration process. To sign up for an event, skiers must contact our office through phone (203-426-0666) or email (). To complete the registration process, skiers must complete the participant registration form, waiver, and deposit process. Please see below for details and contact us if you have any questions. We can’t wait to ski with you!
1) Participant Registration Form:
This should be filled out once by all skiers at the beginning of the year or ski season, as part of the registration process for an event. This form includes contact information, history with adaptive sports, seasonal ski goals, and medical information to be used in the event of an emergency. Any changes to information can be submitted through the office. Participants of the winter program and summer program have separate registrations forms:
2) Waiver: (Available at events)
This should be filled out by all skiers at the beginning of each snow and water ski season. A parent, guardian, or caregiver must sign for participants under the age of 18 and/or participants who are unable to read and comprehend the inherent risks described on the form. Original signed waivers are needed for our records so please bring them to the event. Waivers will also be available at events. No person shall be permitted on the dock or in the water without a properly signed waiver. Our Summer 2020 waiver (updated with COVID19) is located here.
